Notes for Alice Mary Boothby
m. (1) (1914) Sidney Paul Galvin, (2) (1923) Lilliston Powys Lane.

This looks like that birth (FreeBMD):

Surname First name(s) District Vol Page
Births Dec 1893   (>99%)
Boothby Alice Mary St. Saviour 1d 114

(St Saviour is in London.)

In the 1901 census she was with her widowed father Charles J Boothby and her brother George and sister Catherine, in Islington, London.

In 1911 she was an actress on the boards in Warrington in theatrical digs with three other actresses.

Her first marriage corresponds by name with the information for her second marriage:

Surname First name(s) Spouse District Vol Page
Marriages Mar 1914   (>99%)
BOOTHBY Alice M Calvin Cardiff 11a  530   <-- error: ‘Galvin’
Galvin Sidney P Boothby Cardiff 11a  530

(I have reported the Calvin/Galvin, it starts with ‘G’, error to FreeBMD.)
(2016, Apr, TFPL: FreeBMD have changed it to ‘Galvin’)

And here’s their daughter, apparently the only one:

Surname First name(s) Mother District Vol Page
Births Jun 1915   (>99%)
Galvin Dulcie M Boothby Leeds 9b 763

With this looking like Dulcie’s marriage:

Surname First name(s) Spouse District Vol Page
Marriages Jun 1939   (>99%)
Galvin Dulcie M Harman Tendring 4a 2400
Harman Cyril O Galvin Tendring 4a 2400

(Tendring is in Essex)

And this is probably Dulcie and Cyril’s (only) child:

Surname First name(s) Mother District Vol Page
Births Sep 1947   (>99%)
Harman Frances L Galvin Colchester 4a 762

And a Frances L Harman got married in Essex in 1967:

Alice had three children with Herbert Fordwych: Bettina (b.c.1919), William and Judith.  Their births on FreeBMD:

William Dundas was a WW2 casualty on 19th Aug 1942, though the notice on the CWGC site says his parents were of Singapore, which was rubbish as Alice remarried Lilliston a couple of years after William’s birth and then lived in England (apparently):

There are five other possible deaths but only this one in Essex, where her husband had died.

The answer is that this is cartainly her death: the Administration award by the Probate Office was for her two surviving children: Bettina Fordyce and Anthony Powys Lane.
